New Patient Appointment Request Process
To schedule an appointment at either our East Syracuse or Township 5 orthopedic office, call us now at (315) 464-4472. You can also request an appointment online using our interactive appointment request form below. Our system will respond to your request within 48 hours.
Please avoid using this interactive appointment request form if you are requesting an appointment for spine-related issues. Instead, contact our office directly at (315) 464-8620.
Once you have made your initial appointment, you will receive login instructions for the Upstate MyChart patient portal and medical history forms. Please log in and complete the forms electronically. Completing the forms prior to your visit will help expedite the new patient medical history processing.
Be sure to bring all diagnostic imaging or test results that have been ordered or performed by other physicians to your appointment.
Established Patient Appointment Request Process
Please sign in to your Upstate MyChart patient portal to request an appointment and update all forms. If you have not registered on our secure portal or have not received authorization to begin using your portal, call our office to obtain an activation code. You may also utilize the interactive appointment request form below.
If you need to reschedule an existing appointment at either East Syracuse or Township 5, please contact our office as early as possible at (315) 464-4472. Please do not use this form if you have a medical emergency, instead directly call 911 or call us immediately.

What to do After Scheduling Your Appointment
Within 7 days of your upcoming appointment, log into mychart.upstate.edu or click the MyChart Precheck-In tab from our homepage to Pre-Check in for your appointment. You must confirm and fill-in your demographics, insurance information, and all other required patient forms. This will expedite your check-in process on the day of your appointment and save ample time.
On the day of your appointment, enter the building and check in with our front desk staff on the first floor.
Our mask and visitor policy: For the safety of our staff and patients, all individuals entering the practice will be required to wear a mask for the whole duration of their visit. If you do not have a mask, one will be provided to you.
