New Patient Appointment Request Process:
To schedule an appointment at our East Syracuse or Township 5 orthopedic office, call us at (315) 464-8600 or request an appointment online using our interactive appointment request form below. Our appointment scheduler will respond to your request within 48 hours.
Please do not use the interactive appointment request form if you are requesting an appointment for a spine related issue. Instead, contact our office directly at (315) 464-8620.
Once you have made your initial appointment, you will receive login instructions for the Upstate MyChart patient portal and medical history forms to complete electronically. Completing the forms prior to your visit will help speed up the new patient medical history intake process.
Be sure to bring any diagnostic imaging or other test results that may have been ordered or performed by another physician.
Established Patient Appointment Request Process:
Please sign in to the Upstate MyChart patient portal to request an appointment and update forms. If you have not registered for our secure portal or have not received authorization to begin using the portal, call our office to obtain an activation code. You may also utilize the interactive appointment request form below.

What to do after your appointment has been scheduled:
- Within 7 days of your upcoming appointment, log into mychart.upstate.edu to precheck-in or click the MyChart Precheck-In button from our Home page. You can confirm your demographics and insurance information, and complete patient forms. This will streamline your check in process on the day of your appointment.
- On the day of your appointment, when you arrive in the parking lot, click the I’m Here button from our Home page and follow the instructions.
- Our mask and visitor policy: For the safety of our staff and patients, all persons entering the practice will be required to wear a mask for the duration of your stay. If you do not have a mask, one will be provided to you. No visitors will be allowed at this time. If a patient is under 18 or has a physical or mental disability requiring a visitor, only one visitor may accompany the patient. All other parties must wait in their vehicle.
